New Client Forms

These are the forms I ask that all new clients fill out. If you are interested in scheduling as a new client, you have an opportunity to download the forms here, print them, and then fill them out before arriving for our meeting. If you would rather wait - or if you forget the forms on the day of our meeting - I will have blank copies available for you in the waiting area.
